New Patient Registration

The doctors welcome new patients who live within our practice area here

If you are unable to use this form you can download the form below or come to the Surgery to get a copy.
We will ask you to complete a new patient questionnaire.
Children under 16 should be accompanied by an adult wherever possible.
Please notify us of any change of address or telephone number as soon as possible. It is your responsibility to keep us informed.
Medical treatment is available from the date of registration. Please contact reception for further information.
Registering a newborn baby
If you have recently had a baby, you can register them with the practice using the same online registration service or paper registration form shown above.
Please bring your child’s Red Book so we can make copies of any relevant information. The Red Book is the main record of your child’s health, growth and development from birth to age 5.
Once your baby is registered, we will confirm their registration and a member of the team will contact you in due course to arrange the postnatal and baby checks, along with your baby’s first immunisations. We arrange these at 8 weeks so that they can be completed in a single visit.
The local Health Visiting team will also contact you about routine visits after birth.
