The Oaks Partnership has to merged with The Cedars Surgery on 13 November 2023.
We are now Swanley Medical Centre and will keep the names of the three buildings as the historic names – Oaks, Cedars and Hextable. All registered patients will be able to access all of our sites which are listed here so Oaks patients who live closer to Hextable can request an appointment at that site.

The reason for the merger is so we can share systems and processes across our sites to make the practice more efficient. Currently our team have to run 2 computer systems alongside each other, which is proving more and more difficult, and is not an efficient use of resources. This is particularly difficult and time consuming, especially for booking patients in for their vaccinations. Once the systems are merged, there will be just one computer system for all patients. Patients will see no change to their services other than being able to deal with your queries more quickly.
Notification to patients for the digitisation of Lloyd George records
The Oaks Partnership has conducted a project to digitise all of our historic paper based patient medical records commonly known as Lloyd George medical records. Digitising these records enabled better utilisation of practice space, increase security as well as making the full patient record easily accessible at the point of care to clinical staff within the practice.
The digitisation process is almost completed, and paper based records will be securely destroyed. The scanning and destruction of the paper records will follow strict data protection guidelines adhered to by the NHS.
Changes to guidance on Face Coverings in GP Practices
Masks are no longer mandatory in some healthcare settings. As a practice we would like to say a big thank you to our patients over the past months in adhering to face mask wearing whilst at the practice. You may continue to do so if it is your preference. We ask patients who attend with respiratory symptoms to continue to wear them; along with any vulnerable/immunosuppressed patients, who we will seat away from the main waiting room.

We are participating in a new approach to improve access for patients to GP appointments. The aim is to direct patients to the most appropriate healthcare professional, which may be a GP or a pharmacist.
Now, if your symptoms could be resolved by a booked consultation with the pharmacist instead of the GP, you will be given a same-day referral to a pharmacy of your choice. We think this is a good thing. Once you see how great your local pharmacist is – they are highly trained and skilled clinicians experienced in treating minor illnesses – we don’t think you’ll look back.
This will also help us to free up GP appointments for people with more complex health needs and ensure that everyone gets treated at the right time, by the right healthcare professional.

Please download the NHS APP, or other medical apps such as Patient Access, to arrange your repeat medication. You can request medication repeats even when we are closed this way.
For prescription queries, the Oaks now has a dedicated telephone number, which may be accessed from 9am to 1pm each day on 01322 619909.
